How to Apply for Eligibility:
Visit www.cosmeticsurgerygrants.org and apply for consideration of a Grant.

We will provide you with a list of participating surgeons near you.

All candidates must have completed an application form. No incomplete or forged applications will be accepted.
 

Applicants must apply and be accepted into the program prior to the commencement of any surgery.

Ineligibility:
(i) Grants are not available for any “designated health services” that are payable by federal health care programs.

(ii) Any applicant that is actively and currently engaged in any phase of plastic or cosmetic surgery may not qualify for consideration of a Grant.
 

(iii) Applicants may not apply for funding for the purpose of settling prior payments made, or existing payments due, for any cosmetic surgery that has already been completed or is about to be completed.
 

Consultation with a Surgeon:
Ensure you attend for each consultation you schedule.

Selection of Program Candidates:
Applicants will be notified by email, text and regular mail of their acceptance and the terms and conditions for qualifying for their grant award.
 

Awards:
Grants are awarded at the sole discretion of the Organization to any qualified applicant on a percentile basis, (maximums apply) based on the face value of the cosmetic surgery plan that was presented by a participating surgeon of this program.

Eligibility:
Program is open to all individuals that are ages 18 and over as of their application date.

Minors are eligible with the consent of their parent or legal guardians.

Verification of Grant Recipient:
Applicants must show a valid piece of identification when requested.

Delivery of Grant:
The Grant will be deducted from the cost of your treatment plan thereby reducing its cost.

General:
Participation is void where prohibited or restricted by law and subject to all applicable Federal, State / Provincial and local laws, rules, and regulations.
 

If in Canada, the program will be governed by the laws of the province of Ontario, Canada and any applicable Federal laws. Any and all legal actions or claims arising in connection with this program in Canada must be brought in the regional municipality of Toronto, Ontario.
 

We are not responsible for garbled, incomplete, misaddressed, postage-due, or misdirected applications, mail or communications, for errors, omissions, interruptions, deletions, defects, or delays in operations or transmission of information, in each case whether arising by way of technical or other failures or malfunctions of computer hardware, software, communications devices, or transmission lines or data corruption, theft, destruction, unauthorized access to or alteration of application materials, loss or otherwise.
 

Receipt of any grant award is conditioned upon compliance with any and all applicable Federal, State/Provincial, and local laws, rules and regulations.

We are not responsible for electronic communications which are undeliverable as a result of any form of active or passive filtering of any kind, or insufficient space in applicant’s e-mail account to receive e-mail messages.

We disclaim any liability for damage to any computer system resulting from participation in, or accessing or downloading information in connection with, this program, and reserves the right, at its sole discretion, to modify, cancel, terminate or suspend the program should any virus, bug, technical failures, unauthorized human intervention or other causes beyond our control that corrupt or affect the administration, security, fairness or proper conduct of the program. In the event of any such cancellation, termination or suspension, a notice will be posted.
 

We reserve the right, at our sole discretion, to disqualify any applicant from this program or any other offer conducted now or in the future if his or her fraud or misconduct affects the integrity of the program. By participating in this program, each applicant agrees to warrant that s/he is eligible to participate in this program.

By accepting the Grant, applicant / recipient agrees to release us, our directors, employees, officers, and agents, including without limitation, its advertising and promotion agencies, from any and all liability, loss or damages arising from or in connection with the awarding, receipt, and/or use or misuse of grant proceeds.
 

Grant recipients will receive a Grant toward their cosmetic surgery plan. No substitution, cash redemption, assignment, or transfer of proceeds is permitted.

We are only responsible for award delivery; not responsible for award utility, quality, or otherwise.

Taxes, if any, are the sole responsibility of the grant recipient.
 

Caution: Any attempt by an applicant to deliberately damage any website or undermine the legitimate operation of this program is a violation of criminal and civil laws. Should such an attempt be made, we reserve the right to seek damages from any such individual to the fullest extent permitted by law in any court of competent jurisdiction within Canada or the United States.

Additional information:
 

• The consultation is usually free, please check with the clinic to confirm.
   

• The Grant is deducted from the cost of your surgery plan thereby reducing its overall cost. 
   

• You will be financially responsible for the remaining portion of your surgery plan that is owing after the Grant is deducted.
   

• You may be asked to demonstrate that you are able to pay for the portion of your surgery not covered by the Grant.
   

• Following your consultation, you must notify your Program Consultant if you wish to accept the surgery plan. Failure to book your surgery before your deadline may result in the forfeiture or indefinite delay of your Grant.
   

• If you accept treatment, the doctor will require a deposit and you can book your surgery date.
   

• The Grant can only be used at a participating surgeon. We will provide you with the names of each participating clinic near you. You can select any participating clinic you wish.
